In adequate flexibility in running shoes strains the tendon limiting its stretching and causing the tendon to twist and strain. In contrast, a 2008 paper by Timothy Kane, Muhammad Ismail, and James Calder in the UK followed a similar protocol—application of a nitroglycerin patch directly over the most painful area of the Achilles tendon every day for six months—but found no added benefits compared to a control group who received only standard physical therapy. 87 The patients who failed to improve underwent surgery, and afterwards, the researchers inspected samples of the tendons of the patients treated with nitroglycerin. Unlike in studies of rats, they found no evidence for increased activity of nitric oxide.
Cerebral palsy, spina bifida, poliomyelitis and muscular dystrophies are included in neurological associations of Achilles problems in children. Normal walking gait requires three rocker positions of the ankle and foot. The Achilles tendon is required for proper push-off during the gait. In cerebral palsy and spina bifida Achilles tendons are contracted and require surgical lengthening. In polio certain muscles are paralyzed and tendon transfer surgeries are recommended. Progression of muscular dystrophies may require both surgical tendon transfer and tendon lengthening. There is no such thing as Achilles tendonitis. The definition of “-itis” implicates that a part of the body is swollen and inflamed, but the Achilles tendon is incapable of swelling (the bursae – a tiny fluid filled sac between the Achilles and the heel can swell, but that’s different). Instead of Achilles tendonitis, we should call it Achilles tendinopathy. Basically, the tendon is weak and dysfunctional, and thus compromised.
